Extending patients clinical care beyond the walls of the hospital

Vitro Software has included a new Remote Patient Management (RPM) and telemedicine module to its Digital Medical Record in partnership with myPatientSpace, offering patients ongoing clinician led care from the comfort and protection of their own homes, an increasingly important functionality for vulnerable patients in pandemic times.

Both patients and hospitals will benefit from the remote management, as non-critical hospital visits can be avoided and social distancing compliance will be easier, particularly in hospitals with reduced capacity due to COVID-19 restrictions. And it is expected that many patients’ recovery journeys can improve, empowering patients and with the added benefit of less travel required for their appointments.

In addition, the RPM module means that a broad cohort of patients, including those with chronic disease, or who are pre- and post-surgery, can now play a more active role in their own care through engaging with pre- and post-operative care programme and educational material. This includes following specific care pathways, recording their progress on an easy to use tool and requesting telemedicine consultations through the myPatientSpace App which is downloadable from the iPhone or Android App stores.

The relevant clinical teams can provide care remotely, monitor the patient's progress and tailor their care pathway, or provide educational material according to the patient's needs. Once a patient documents their outcomes via the mobile app, the information is fully integrated into the clients Digital Medical Record at the hospital, this means that clinicians have the patient’s complete medical record in one place, on Vitro.

During telemedicine interactions the clinician continues to use Vitro. They can easily scroll through the patient's digital chartbook reviewing recent or historical episodes which could include information such as observations, progress notes, surgical records, and laboratory or radiology results etc. The clinician will continue to record the telemedicine interaction as a new episode in Vitro.
